ESSENTIAL JOB FUNCTIONS
Patient Care Provides healthcare to patients in an out-patient setting. Works under the supervision of the physician and in accordance with written protocol. Responsibilities include: · taking histories performing physical examinations · forming diagnostic and treatment plans · reviewing cases with the physician in the delineation of standard treatment regimens. Healthcare Provides healthcare which consists of promotion and maintenance of health, prevention of illness and disability, basic care during acute and chronic phases of illness, guidance and counseling of individuals and families and referral to other health care providers when appropriate. Patient Assessment· Assess the physical and psychological status of patients by means of interview, health history, physical examination and diagnostic tests. · Makes assessments of actual or potential health problems and observes signs and symptoms associated with changes in a patient's physical and emotional health. · Evaluates test findings, makes assessments and initiates appropriate actions to facilitate the implementation of preventative and/or therapeutic plans consistent with the continuing health care needs of the patient. Documentation completes all required documentation of services in an accurate, complete, and timely manner following hospital and departmental policies and procedures. Physician Interaction Establishes interpersonal relationship with the clinic's physician, and other health care team members, thereby facilitating a free flow of essential information necessary for the patient's healthcare needs. Provides Information Reports pertinent information regarding any problems in providing clinic services, personnel problems, and any unusual occurrences to the clinic director. Department Programs Recommends short and long range plans and programs, policies and procedures to obtain objectives. Recommends to supervisor material, equipment, and services to be used by the clinic. Orientation Provides department orientation to newly employed personnel and continued guidance and sharing of knowledge with co-workers in areas of new skills, techniques, procedures and policies. Performance Improvement Participates in the performance improvement efforts of the hospital. Produces and monitors related department quality indicators. Committees Serves on hospital committees as requested by supervisor Other Duties Performs other related duties as required.

Takes appropriate steps to learn about each patient such as asking questions to better understand the cultural influence and help the patient feel better about their care.

Uses available resources such as interpreter, patient family members, dietitians, other health care workers, etc. to assist in providing appropriate care while maintaining respect for patient's cultural beliefs.
KNOWLEDGE, SKILLS AND ABILITIES
Licensure in the State of New York as a Nurse Practitioner, or evidence of eligibility for same required.
Requires demonstrated ability to lead and direct others along with good organizational skills to effectively operate an outpatient clinic.
A minimum of two years experience as a Nurse Practitioner required.
Must possess good communication skills to effectively interact with patients, their family members, physicians, and co-workers.
WORKING CONDITIONS
Patient care areas where there is no physical discomfort due to temperature, lighting, noise, dust, etc. Work requires prolonged walking and standing (over 90% of work time). Some exposure to unpleasant sights and odors. May assist in lifting and moving patients. Potential exposure to blood and body fluids.
